MISSION STATEMENT
Patient PLUS Homehealth Care, Inc. has been established for the purpose of providing quality home health care service. The agency is committed to the ideal that individuals and families have the right to participate in the decisions regarding their own health care and the way in which care is provided. The agency's mission is to provide families and individuals with a safe and effective source of quality home health care services.

Our agency's focus is to promote and maintain the physical, emotional, spiritual, and psychosocial health of our patients; thus providing a holistic and multidisciplinary approach to health care in the home. The agency strives to work effectively with other health care providers and community organizers to promote continuity of care.

COMMUNICATION WITH SENSORY IMPAIRED PERSONS
For any client requiring TTD or TTY devices as a method of communication, Patient PLUS will contact the Illinois Telecommunications Access Corporation (ITAC) to arrange for their service.

For any client with limited English proficiency, Patient PLUS can provide interpreters at no cost.

ELIGIBILITY
To qualify for care, a patient must:
 - be essentially homebound. Need not
   be bedridden but needs assistance to
   leave home
 - need intermittent care
 - require skilled medical care by
   healthcare professionals

MEDICARE, Medicaid, and commercial insurance (for qualified clients) cover home health services.
